Later it should be submitted at the concerned Mandal Revenue Office after duly filling and enclosing the following papers. They are : | |
Two forms of caste certificates given by two gazetted officers separately | |
An affidavit duly notarized given on Rs. 10/- bond paper | |
A Xerox copy of transfer certificate or bonafied certificate duly attested by a gazetted officer | |
Xerox copy of ration card duly attested by a gazetted officer. | |
A Xerox copy of Electronic photo ID card (voter ID card) given by Election commission of India | |
Caste certificate of any family member of applicant duly attested by a gazetted officer or original caste certificate given by the applicant’s caste elders association | |
Two passport size photos | |
Phone No. Residence & Mobile
