How do i get a GHMC Death Certificate at Hyderabad?
  • If any natural death occurs at any hospital, such hospital authority has to inform the nearest GHMC ward office about the details of the death in form No.2 within 21 days
  • If it is unnatural death the concerned Station House Officer of the local police station has to be informed to the nearest GHMC ward office about the details of the death in form No. 2 within 21 days
  • Later you should approach the ward office for death certificate on any working day between 3 to 5 pm
  • You can get the GHMC death certificate from the concerned ward office within three (3) working days
  • If there are any mistakes in the GHMC death certificate such corrections can be made by producing the following documents.
  • Declaration should be given by two Gazetted Officers (The names of the Gazetted Officers are to be written in capitals)
  • An affidavit duly notarized on Rs. 10/- non judicial stamp paper.
  • Original GHMC birth certificate to be returned
  • Documentary evidence like Education Certificates, voter ID Card, Ration Card, Passport or any other registered documents. But the documents like PAN Card, LIC and Bank Statements are not accepted
