GHMC Death Certificate for Unregistered Deaths

The following procedure has to be followed for Deaths which have not been registered. It is mostly applicable for cases where the Death has taken place many years back to get GHMC Death Certificate.

Procedure to be followed:


I)Walk into any eSeva center and fill in the application form available at eseva for Non-availability Certificates of death. Based on your details eseva operator will verify the data whether the death data is already registered?

II)If the record not found, applicant has to apply for non-availability certificates.

III)The following documents are required along with the filled application form.

Non-availability GHMC Certificates of Death:
1. Ration card/any other residence proof.
2. Notarized affidavit on Rs 10/- Non-Judicial Stamped paper.
3. In case of Medico-legal cases.
a) FIR
b) Post Mortem Report
c) Form-2 given By Concerned Police Authorities
4. Any other documents if any.

How do i get a GHMC Death Certificate at Hyderabad?
  • If any natural death occurs at any hospital, such hospital authority has to inform the nearest GHMC ward office about the details of the death in form No.2 within 21 days
  • If it is unnatural death the concerned Station House Officer of the local police station has to be informed to the nearest GHMC ward office about the details of the death in form No. 2 within 21 days
  • Later you should approach the ward office for death certificate on any working day between 3 to 5 pm
  • You can get the GHMC death certificate from the concerned ward office within three (3) working days
  • If there are any mistakes in the GHMC death certificate such corrections can be made by producing the following documents.
  • Declaration should be given by two Gazetted Officers (The names of the Gazetted Officers are to be written in capitals)
  • An affidavit duly notarized on Rs. 10/- non judicial stamp paper.
  • Original GHMC birth certificate to be returned
  • Documentary evidence like Education Certificates, voter ID Card, Ration Card, Passport or any other registered documents. But the documents like PAN Card, LIC and Bank Statements are not accepted

  • For already registered death, they can request for GHMC death certificate in any of the e-Seva Centers in twins cities or at www.esevaonline.com
  • Walk into any eSeva center and fill in the application form available at e-seva for death certificate request. Based on your details e-seva operator will verify the data whether the death is already registered? You can also download the application form from here.

  • If registered, pay the required fee amount of Rs.20/ per certificate and e-Seva service charges Rs.15/- and required courier charges as per the option below


  • Applicant can choose the option for delivery of GHMC death certificate: 


1) At eSeva Rs.15/- 
2) Within GHMC Limits Rs.25/- 
3) With in AP (out side of GHMC) Rs.40/- 
4) With in India Rs.60/- 

  •  If the applicant chooses the option 1, applicant has to come to eSeva for collecting the certificate/s after 3 working days(excluding the date of application).


  •  If option 2 or 3 the certificate/s will be courier to applicant address after receiving the certificate/s from the GHMC Circle offices.
